Terms and conditions

We, the parents/guardians of aforementioned minors, in consideration of the benefits to our child in her participation in the Purpose & Purity Conference, do hereby consent to our child’s participation in activities under the jurisdiction of the organizers of this event. I, the undersigned, do hereby release and agree to hold harmless the I Am Purposed Organization, it’s event organizers, volunteers and sponsors from any and all liability, claims or demands for accidental personal injury or sickness, as well as property damage and expenses, of any nature whatsoever which may be incurred by the undersigned and the Participant while involved in the event.

In addition, the I Am Purposed Organization has my permission to use my or my child’s photograph or video image publically to promote the organization. I understand that the images may be used in print publications, online publications, presentations, websites, and social media. I also understand that no royalty, fee or other compensation shall become payable to me by reason of such use and waive any right to inspect and/or approve the copy that may be used in connection therewith, wherein my child’s likeness appears, or the use to which it may be applied.

If during a period of time, when my child is in the care, custody and control of the event’s organizers or volunteers, my child should come to need emergency medical care, I hereby authorize said persons to obtain the necessary medical aid, and such additional operations or procedures as considered necessary or desirable in the judgment of the doctor consultant for medical treatment, provided that we the parents/guardians, are notified at the earliest possible time of the injury to my child, her medical condition, and the necessity of the medical treatment.
